FALL REGISTRATION:

Football and Tennis athletes for the 2024-2025 school year must submit athletic paperwork by July 1st. All other fall athletes must submit forms by August 1st.

WINTER REGISTRATION:

All winter athletes must submit forms by November 1st.

SPRING REGISTRATION:

All spring athletes must submit forms by February 13th.

SPORT SIGN-UP BY PARENT: parents are required to sign-up their child for their desired sport on Genesis

1) Activity Fee: In accordance with Board policy #2436, students who participate in school-sponsored co-curricular activities are subject to a participation fee. The fee is paid only once per year; a student who has paid the fee may participate in as many athletic/extracurricular activities during the course of the school year as they would like without making additional activity fee payment. Please follow this link to the policy. Any student participating in Chatham High School athletics is subject to the activity fee. Click here to pay the registration fee.

2) Updated Preparticipation Physical Evaluation Form  and History Form 

Physicals are valid for 365 days. Starting July 1st, the Athletic Pre-Participation Physical Exam Packet will remain with your child’s physician and is no longer submitted to the CHS school nurses. 

You may view the date of your child's last physical by visiting the Athletics tab within the Parent Portal.

3) Preparticipation Physical Evaluation Medical Eligibility Form

This form will also be completed by your child’s physician and is required to be submitted to the CHS school nurses.

This form will then be submitted to the CHS school physician for final clearance.

4) Health History Update Form 

This form will be required if your child’s physical was completed over 90 days prior to the start of the season and is required to be submitted to the CHS school nurses.

5) Digitally Sign Electronic Forms: The following forms are available in the Parent Portal. To sign the forms electronically, sign into the Parent Portal, select the "Forms" tab, and select "Athletic Release [YEAR]." Check off all of the check boxes that apply.  DO NOT submit these forms in hard copy if you have signed off on them in the Parent Portal.

5) Review NJSIAA List of Banned Substances: Each year, NJSIAA updates the list of Banned Substances. Please ensure you are aware of the banned substances by clicking on this link:  2024-2025 NJSIAA Banned Substances 

Your child WILL NOT be able to participate unless every form is filled out and submitted properly.
