Information & Program Requirements
Please read the program description and requirements before considering applying.
This document must be completed by the coach for your selected program and uploaded as part of the application.
Before beginning this application, be sure that you have these things ready for upload:
A written descriptive outline (brochure) of the program
A completed "Coach's Verification Form" (This can be downloaded from the CHS webpage)
Confirmation of your registration in the program (A receipt that documents payment or, if payment has not yet been made for the entire year, a letter from the program documenting your registration in the program).
